You can use the IRS for the following types of incident:

A fatality

An incident where someone has gone directly to hospital from your premises.

A serious injury to an employee as a result of an accident or assault.

A serious injury to a school pupil

A premises-related issue where there is serious and imminent danger to employees, pupils, customers etc.

Serious injuries can include (but are not restricted to):

Fractures

Amputations (other than fingers or toes)

Loss of consciousness caused by head injury

Serious burns

Crush injuries

Injuries which are likely to lead to permanent loss of/reduction in sight

Injuries likely to result in at least 7 days lost time from work

Please do not use this function to report minor incidents, or those which are not work-related.
